SGLT2 抑制剂类糖尿病药物研发现状

    钠-葡萄糖共转运蛋白 2 (SGLT-2) 抑制剂是相对较新的一类口服降糖药。葡萄糖易化转运体 (GLUT)与SGLT 是人体内参与葡萄糖吸收和重吸收转运体的两大家族。其中SGLT 家族中代表性的成员是SGLT-1 和SGLT-2。SGLT-1 主要分布在小肠、肾脏、心脏和脑内,其主要生理功能为在小肠完成对葡萄糖的吸收。SGLT-2 特异性的分布在肾脏近曲小管S1 部位,负责约 90%葡萄糖的重吸收,剩余10%由位于近曲小管S3 部位的SGLT-1 完成。SGLT2 抑制剂通过特异性地阻断肾脏对葡萄糖的重吸收,通过尿排出多余的葡萄糖,从而有效地降低糖尿病患者血糖水平。

    目前在SGLT2 抑制剂领域研发较为活跃的公司是辉瑞、勃林格殷格翰、默沙东、百时美施贵宝和阿斯利康,这几家公司目前均有3 个在研的SGLT2 抑制剂,强生公司则拥有两个,其余如赛诺菲、诺华、安斯泰来等多家公司目前拥有1 个SGLT2 抑制剂在研药物。

简要评述

    尽管在2012 年, dapagliflozin 在美国上市受挫没,但是有报道称百时美施贵宝正与FDA 保持沟通,计划今年内再度提交上市申请。如果顺利的话,勃林格殷格翰与礼来公司共同开发的empagliflozin 也会在短期内在美国和欧洲上市。另一个值得关注的SGLT-2 抑制剂是默沙东和辉瑞公司共同开发的ertugliflozin,它单独治疗的方案将会在今年晚些时候进入Ⅲ期临床。

      作为相对新颖的作用机制的糖尿病治疗药物,SGLT-2 抑制剂在临床上表现出一些令人意想不到的积极的和负面的效应,主要包括降低体重、增加尿路感染的风险、降低血压、增加脱水症的风险以及高钾血症等。正是这些意外的正面或负面的效应将在很大程度上影响SGLT-2 抑制剂的上市和销售前景。例如,考虑到糖尿病人很多面临着肥胖的问题,尤其是注射胰岛素导致进食增加,SGLT-2
抑制剂能在很大程度上抵消由此导致体重增加。但是尿路感染、对血压的影响等负作用增加了医生和患者使用SGLT-2 抑制剂的疑虑。目前,糖尿病发病率不断攀升。尤其是考虑到糖尿病是一种渐进性疾病,患者在长期用药的过程中有淘汰老药换用新型药物的需求,新的作用机制的药物往往会具有很大的市场前景。SGLT-2 抑制剂也不例外。百时美施贵宝的发言人指出,尽管市场上有多种作用机制的许多糖尿病药物,但是目前许多2 型糖尿病患者很难达到或维持糖化血红蛋白指标,这样巨大的未被满足的医疗需求为新作用机制的糖尿病药物提供了巨大市场需求。因此,可以认为SGLT-2 抑制剂是很有市场前景的一类糖尿病药物。但是由于作用机制较为新颖,大规模临床数据相对较少,在开发此类药物时,应当密切关注目前在研药物临床进展以及已上市药物的上市后研究。
